"NOPQRST!" Screamed speaker-B.
Aaaand string of italic text are usually thoughts. When it's just a word that is italic, it's just an emphasis on that particular word.
For example:
She sobbed into her hands. I'm in deep . (THOUGHTS)
She's couldn't believe her ears. (EMPHASIS - may be exaggeration, accents, sassytalk, etc. )
