google docs is really not that professional. Formassembly, I take the free trial but it has an expiry date. Typeform is the best!

I use typeform which is free unless you want premium, but everything necessary is available for free. You can make different types of fields and have multiple forms all in one place. I find it very useful. You also have some liberty with how your form looks (themes and colors and images) with the option to add your own images.

you could try Formassembly, I've used it before and it works like a charm ;)

Google docs is free and comfortable to use. I use it and it's quite good to me till now.
