How to fill out the form:

There are groups of subjects in a column down this blog post.

Beside the subject name, if it has a...

1/ [o] means rank - 1 being your favourite subject and the lowest number being the least. You replace the 'o' with a number. If your character hates one subject so she would just not do it, put a 0 (zero) instead of a number. 

2/ [x] means choice - you have to choose which subject you would like out of the given choices. You erase all 'x's' apart from one and that one 'x' marks the subject your character would like to take.

If you want, you can also write a sentence or two about why your character likes/hates that particular subject beside the [o] or [x].
