NotesPatient: Kim, Taeyeon
I have finished my evaluations of the patient and have concluded that she is completely rehabilitated. I discovered that the patient was only afraid of the unknown, but once I got her to address her issues directly, she was able to overcome them easily. I have had a conference with Taeyeon's guardian where it was reported that Taeyeon's behavior has improved and she appears not to struggle with her temper currently. I will reduce the amount of required visits, though Taeyeon may come in whenever it is deemed necessary at her discretion. I will continue to make periodic check ups with Taeyeon to be sure that she is managing her anger well or any new personal issues that may arrive.
I have arrive at the decision that Taeyeon be removed from the program, and cease all prescriptions as they are no longer needed and prolonged use could be damaging.
Seeing how Taeyeon is removed from my treatment, visits are not required and I will refer to her as a patient no longer. However, Taeyeon and Ms. Kim may visit my office at any time, free of charge, to avoid Taeyeon having to be re-admitted to my program. I ask that Taeyeon continue to use the exercises I have taught her and keep the notebook I have given her, but the daily logs are optional.
