putting other names, birthplace and nationality when there is already a section for birth name, hometown and ethnicity seemed redundant. i am well aware that those things aren't the same but i can't help but feel like the application can do without those sections. but if you feel that you must add the other names, birth place and the nationality of your character feel free to add it under the trivia section.
photos within the form
change the photos of seongwu into the photo's of your character's faceclaim and replace the one of minhyun with your character's love interest.
